WHICH ARE THE DOCUMENTS DO THE INTENDED PARENTS NEED TO BRING WITH THEM?
Set of legal documents needed for the initiation of our surrogacy programs:
For Georgia:
Married or unmarried heterosexual couple that has been married or in cohabitation for at least 1 year
Original Certificate of Marriage or rent agreement or other document that confirms the cohabitation of the Intended Parents for 1 year
Valid Passports
Your original marriage certificate or cohabitation certificate must be stamped with apostille (or legalization for countries that do not support apostilization).
No age limit
For Armenia:
Married heterosexual couple (no minimum amount of time since marriage is required) or
Single Woman with own oocytes or
Single man (if he is Armenian by origin or citizenship)
Original Certificate of Marriage (if a married couple)
Valid Passport(s)
Your original marriage certificate must be stamped with apostille (or legalization for countries that do not support apostilization).
The maximum age of the Intended Mother is 52 years old
For Belarus:
Married heterosexual couple (no minimum amount of time since marriage is required) or
Single Woman with own oocytes or
Original Certificate of Marriage (if a married couple)
Valid Passport(s)
Your original marriage certificate must be stamped with apostille (or legalization for countries that do not support apostilization).
The maximum age of the Intended Mother is 49 years old
NECESSARY MEDICAL TESTS TO BE COMPLETED BEFORE THE TRIP:
Intended Father:
Blood group and Rh;
Spermiogram;
Fragmentation test;
Test hepatitis B and C;
HIV test (IV generation antibodies + p24 antigen);
TPHA- Syphilis test;
Intended Mother (ONLY if her own oocytes are going to be fertilized):
